The third stage (24 to 72 hours) primarily affects the renal system, with symptoms like flank pain, hematuria, and acute kidney failure due to the deposition of calcium oxalate crystals.

Laboratory tests are essential for diagnosis, including serum ethylene glycol levels, blood gas analysis to assess metabolic acidosis, and measurement of anion and osmolal gaps. The presence of calcium oxalate crystals in urine is a strong indicator of poisoning. Advanced diagnostic tools like gas chromatography can accurately measure ethylene glycol levels in the blood.

Treatment

The primary goals are to inhibit further metabolism of ethylene glycol and correct metabolic acidosis.
