On June 18, 2023, the submersible Titan imploded in the North Atlantic while descending toward the wreck of the Titanic, killing all five people aboard within moments. What began as a high-profile deep-sea expedition became one of the most scrutinized maritime disasters in recent memory, with investigations ultimately concluding the tragedy was entirely preventable.
The Dive That Went Wrong
Titan imploded in the Atlantic Ocean at a depth of roughly 3,500 meters, or about 11,482 feet, as it descended toward the Titanic wreck site. The submersible went down on its 88th dive. Its disappearance triggered a frenzied, multinational search operation that stretched on for days before the debris field was finally located on the ocean floor. That search captured global attention, with search-and-rescue teams racing against a shrinking oxygen supply before it became clear the vessel had already been destroyed.
Who Was Aboard
The five people killed were Stockton Rush, the 61-year-old co-founder of OceanGate; Hamish Harding, a 58-year-old adventurer and owner of Action Aviation; Paul-Henri Nargeolet, a 77-year-old veteran Titanic explorer from France; British-Pakistani businessman Shahzada Dawood, 48; and his 19-year-old son, Suleman. Nargeolet in particular was widely known as a Titanic expert, having dived to the wreck many times before joining this fateful expedition.
What Investigators Found
Multiple official investigations followed the disaster, and their conclusions were remarkably consistent. The Coast Guard convened its highest level of inquiry and ultimately determined the implosion was “preventable.” Investigators found that Titan’s design, certification, maintenance, and inspection process were all inadequate, and that OceanGate operated with a “toxic workplace culture,” with its mission further hindered by a lack of domestic and international regulatory framework for submersible operations.
The Coast Guard’s report identified OceanGate’s failure to follow established engineering protocols for safety and testing as the primary cause, describing the company’s safety culture and operational practices as critically flawed, with major gaps between its written safety protocols and what it actually did in practice. Investigators found OceanGate continued using Titan even after a series of incidents had compromised the hull and other critical components, without properly assessing or inspecting the craft afterward.
The National Transportation Safety Board reached similar conclusions. Its report, released in October, blamed the company for shoddy design and a failure to adequately test the vessel before its fateful June 2023 trip, pointing to an inadequate engineering process and damage that went unnoticed.
A third investigation, from Canada’s Transportation Safety Board, added further detail after examining the incident since Titan had launched from the Canadian research vessel Polar Prince. Canadian investigators determined that the as-built properties of Titan’s carbon fiber cylinder were never validated against the theoretical values used in its design, and that the vessel’s construction and testing didn’t follow standard engineering practices. The report also identified structural flaws tied to the hull’s shape and material—it was cylindrical rather than the industry-standard spherical shape, and built from carbon fiber instead of titanium, which is the conventional choice for deep-diving submersibles.
An Unconventional Design
Titan’s construction broke sharply from established practice in deep-sea engineering. Its pressure hull consisted of a carbon fiber cylinder capped at either end by titanium domes—a novel approach for a human-occupied submersible intended for deep-ocean diving, since submersibles built for that purpose are typically made entirely of steel or titanium.
Warning Signs That Went Unheeded
Testimony gathered during the investigations suggested the risks weren’t unknown beforehand. A transcript from a 2018 OceanGate meeting revealed that the company’s CEO said at the time, “No one is dying under my watch, period.” Investigators also examined a loud bang heard during a 2022 mission and its potential structural consequences for subsequent dives, raising questions about whether the vessel’s integrity had already been compromised before its final descent.
The Aftermath
The disaster’s ripple effects extended well beyond the immediate loss of life. It has led to lawsuits and calls for tighter regulation of the developing private deep-sea expedition industry. Had Stockton Rush survived, the Coast Guard’s investigative team said it would have recommended manslaughter charges to the Department of Justice. Canada’s Transportation Safety Board issued six recommendations addressing regulatory oversight, technical standards for submersibles, and safety management deficiencies, and regulators in multiple countries have since moved to tighten oversight of an industry that had largely operated in a gray area of maritime law.
