Post-exposure prophylaxis (PEP) is highly effective at preventing HIV infection after a potential exposure, but it isn’t foolproof. While true PEP failures are rare, understanding why they happen — and what’s more often mistaken for failure — matters for anyone who has taken or is considering PEP. Here’s what the evidence shows.
What PEP Is and How It Works
PEP is a 28-day course of HIV medication taken after a possible exposure to the virus, whether through unprotected sex, needlestick injuries, or other routes. To be effective, it must be started within 72 hours of exposure, and the sooner it begins, the better the chances of preventing infection. It’s designed strictly as an emergency measure, not a substitute for regular prevention methods like PrEP or condoms.
Most “PEP Failures” Aren’t Actually Failures
One of the most important findings from research on PEP outcomes is that the majority of HIV infections diagnosed after PEP use aren’t caused by the medication failing — they’re the result of continued risk exposure after completing the course. In one notable analysis of HIV infections following PEP, the large majority of cases were linked to ongoing risk behavior after PEP was finished, with many diagnoses occurring more than six months after starting treatment, well beyond the window PEP could plausibly have influenced. When researchers narrowed the data down to cases that might genuinely represent treatment failure, the number was small, working out to roughly 5 transmissions per 1,000 PEP users.
Genuine Causes of PEP Failure
When PEP does fail to prevent infection, a handful of factors tend to be responsible:
- Delayed initiation. PEP’s effectiveness drops the longer the gap between exposure and starting treatment, and starting outside the 72-hour window significantly reduces its protective value.
- Poor adherence. Missing doses or not completing the full 28-day course reduces the medication’s ability to fully suppress the virus during the critical window after exposure.
- Drug-resistant HIV strains. In rare documented cases, infection occurred because the source’s HIV strain was resistant to one or more drugs in the PEP regimen, making the treatment less effective despite proper use.
- High viral inoculum. In at least one documented occupational case, a large amount of HIV-infected blood was involved in the exposure, which may have overwhelmed the level of protection PEP could provide.
Why Adherence and Timing Matter So Much
Across multiple studies, adherence to the full PEP course consistently comes up as one of the strongest predictors of successful prevention. Some PEP regimens are easier to stick with than others, and patients on more tolerable drug combinations tend to complete treatment more reliably. Since PEP relies on maintaining sufficient drug levels in the body throughout the exposure window, inconsistent dosing gives the virus more opportunity to establish infection.
What This Means in Practice
For most people, PEP remains a highly effective emergency intervention when started promptly and taken exactly as prescribed. The rare instances of genuine failure tend to involve identifiable risk factors — delayed start, missed doses, or drug-resistant virus — rather than the medication simply not working. Continued risk behavior after finishing a PEP course is a far more common explanation for later HIV diagnoses than treatment failure itself, which is why healthcare providers typically emphasize both completing the full course and adopting ongoing prevention strategies afterward, such as PrEP for those at continued risk.
