Any pain typically wanes within 30 minutes, and a small area of goosebump-like skin irritation, roughly the size of the jellyfish itself, may appear and fade shortly after. This deceptively mild start is part of what makes Irukandji stings so dangerous — victims often don’t realize anything serious happened until symptoms hit later, sometimes after they’ve already left the water.
How Fast Symptoms Escalate
Severe systemic symptoms usually begin within 30 minutes of the sting, though the onset can range anywhere from 5 minutes to 2 hours. Once they start, the symptoms are severe and wide-ranging:

The antivenom developed for the larger box jellyfish species has not shown effectiveness against Irukandji syndrome specifically, and beta-blocker medications are avoided due to the risk of causing dangerously low blood pressure on top of the syndrome’s existing cardiovascular strain. Deaths from Irukandji stings are rare, but they do happen, and box jellyfish as a family have been linked to more than 60 deaths in Australia over the past century. The risk of a fatal outcome depends on factors like how much venom was injected, the victim’s overall health, and how quickly they receive medical care, with children, older adults, and people with existing heart conditions facing higher risk from the syndrome’s cardiovascular effects.
